68 <br />Standard Grant Template Dated 1/242012 <br />Grant Agreement Number _ _y,6.z_ _ <br />Between the Minnesota Department of ealth and City of St. Anthony Village <br />Minnesota Department of Health <br />Grant Agreement <br />This grant agreement is between the State of Minnesota, acting through its Commissioner of the <br />Department of Health ("State") and City of St. Anthony Village ("Grantee"). Grantee's address is 3301 Silver <br />Lake Rd. NE, St. Anthony, MN 55418-1667:. <br />Recitals <br />1, Under Minnesota Statutes §114D.50 Clean Water Fund, the State is empowered to enter into this Grant <br />Agreement. <br />2. The State is in need of assisting public water suppliers to implement their source water protection plans. <br />3. The Grantee represents that it is duly qualified and will perform all the duties described in this agreement to <br />the satisfaction of the State. Pursuant to Minnesota Statutes § 1613.98, subd, 1, the Grantee agrees to minimize <br />administrative costs as a condition of this grant. <br />Grant Agreement <br />1. Terrn of Agreement <br />1.1 Effective date June 15, 2012, or the date the State obtains all required signatures under Minnesota <br />Statutes § 16C.05, subd,, 2, whichever is later. <br />The Grantee must not begin work until this contract is fully executed and the State's Authorized <br />Representative has notified the Grantee that work may commence. <br />1.2 Expiration date June 30, 2013, or until all obligations have been fulfilled to the satisfaction of the <br />State, whichever occurs first. <br />1.3 Survival of Terms The following clauses survive the expiration or cancellation of this grant contract: 8. <br />Liability; 9. State Audits; 10.1 Government Data Practices; 10.2 Data Disclosure; 14.1 Publicity; 14.2 <br />Endorsement; and 15. Governing Law, Jurisdiction, and Venue. <br />2. Grantee's Duties The Grantee, who is not a state employee, shall: <br />- Complete to the satisfaction of the State all of the following duties: <br />- Conduct well inventory and distribute educational materials to property owners at the time of <br />the inspection <br />- Mail survey materials to property owners of parcels with wells within the DWSMA <br />- On or before the end date of this Agreement, the Grantee shall provide the State with one electronic <br />copy of all final products produced under this Grant Agreement, including reports, publications, <br />software and videos. Any Grantee activity that involves data collection must be submitted to the State <br />upon completion of the project and in the format agreed by the State. <br />- The Grantee shall pay in full any contractor hired for the purpose of completing the work under this <br />Page 2 of 8 <br />
