Access Solutions was awarded a grant for a two-year period <br />to provide home modification services to older adults in St. Anthony Village and <br />Roseville to facilitate and promote aging in place. Access Solutions is <br />collaborating on this project with staff within each city, Greater Metro Housing <br />Corporation's Housing Resource Center and other local businesses to identify <br />community participants, identify additional funding resources and services as <br />necessary. <br />General Scope of Project <br />a. Review Proposal Narrative (handout) <br />b. General Objectives <br />i. Enhance the quality of life for older adults living in their own <br />homes in St. Anthony Village and Roseville. <br />1. Provide home safety assessments and recommendations <br />to 25 older adults in each city over the course of two years. <br />Each grant participant will then receive project <br />management assistance and up to $500 to implement <br />home modifications in to their home. <br />2. Provide information and resources to participants on <br />additional services that promote aging in place. <br />3. Provide assistance to participants for identifying and <br />securing funding resources for implementing aging in place <br />modifications. <br />Educate consumers and staff at each city and GMHC in universal <br />design and aging in place <br />1. Provide outreach at a variety of community settings <br />2. Provide educational materials to industry professionals <br />2. Communication/marketing <br />a. Identify key contacts at each city such as: <br />i. Housing Authority <br />ii. Senior/Community Center <br />iii. Other <br />b. Identify existing outlets for contacting older adults <br />3. Questions <br />
