at s)(%,f AUG US'T'ANA THERAPY SERVICES 21 ACCESS SOLUTIONS 1007 E. 141" Street Minneapolis, MN 55404 Ph 612.238.5429 F 612.339.3085 SAV Council Meeting 08.28.12 Access Solutions, a division of Augustana Care, provides home safety assessments, universal and accessible design consulting and project management services to promote safe and independent living for people of all ages and abilities. Access Solutions was awarded a grant for a two-year period to provide home modification services to older adults in St. Anthony Village and Roseville to facilitate and promote aging in place. Access Solutions is collaborating on this project with staff within each city, Greater Metro Housing Corporation's Housing Resource Center and other local businesses to identify community participants, identify additional funding resources and services as necessary. General Scope of Project a. Review Proposal Narrative (handout) b. General Objectives i. Enhance the quality of life for older adults living in their own homes in St. Anthony Village and Roseville. 1. Provide home safety assessments and recommendations to 25 older adults in each city over the course of two years. Each grant participant will then receive project management assistance and up to $500 to implement home modifications in to their home. 2. Provide information and resources to participants on additional services that promote aging in place. 3. Provide assistance to participants for identifying and securing funding resources for implementing aging in place modifications. Educate consumers and staff at each city and GMHC in universal design and aging in place 1. Provide outreach at a variety of community settings 2.
