s.hv: ttovu.e fora U* feti,wKe: <br />Aging in Place Project <br />.n <br />, �!Y - Al. :** <br />&Centro ACCESS SOLUTIONS <br />RE11A0* <br />What is the SAV: Home for a Lifetime — Aging in Place Project? <br />Through funding by a grant from the MN Department of Human Services: Access <br />Solutions, GMHC(Greater Metropolitan Housing Corporation) and the City of St. <br />Anthony Village have partnered to provide information and outreach to consumers to <br />provide resources and support to older adults to continue living independently by <br />implementing simple, cost effective home modifications to lower the risk of falls and <br />promote active living. <br />General Objectives include: <br />1. Enhance the quality of life for older adults living in their own homes in St. Anthony <br />Village <br />a. Provide home safety assessments and recommendations to 25 older adults in <br />each city over the course of two years. Each grant participant will then <br />receive project management assistance and up to $500 to implement home <br />modifications in to their home. <br />b. Provide information and resources to participants on additional services that <br />promote aging in place. <br />c. Provide assistance to participants for identifying and securing funding <br />resources for implementing aging in place modifications. <br />2. Educate consumers and staff at each city and GMHC in universal design and aging <br />in place <br />a. Provide outreach at a variety of community settings <br />b. Provide educational materials to industry professionals <br />What is Aging in Place? Aging in Place is the ability to live in one's own home — <br />wherever that might be- for as long as safely, confidently and comfortably possible. <br />Livability may be enhanced and extended through the incorporation of services and <br />features, such as universal design. <br />What is Universal Design?
