Enclosed is the <br />Statewide Health Improvement Program (SHIP) physical activity partner project service <br />contract. We are extremely pleased to be pursing an agreement with you to work on the <br />strategies for active living in your community. <br />In order to receive the first installment of your funds, please follow the steps below: <br />• Review the final documents which have been approved by the Washington County <br />Attorney's office. <br />• An official signature is required on page two of the service contract (flagged under <br />"vendor"). <br />• The signed agreement, Attachment A, certificates of insurance and completed invoice can <br />be sent to the Department of Public Health and Environment, c/o Jean Streetar, Program <br />Manager. <br />• Once everything is complete, the documents will be signed by the county and a copy sent <br />to you for your files. The first installment of funds will be sent shortly after that time. <br />• Pursuant to the terms of the agreement. The second installment will be provided when the <br />deliverables, evaluation form and invoice are completed.
