Reference the SHIP funding source as required by MDH for all material and events. <br />6. Reference Washington County and "Living Healthy in Washington County" for all <br />materials and sponsored events. <br />7. Comply with lobbying requirement provided by MDH. <br />8. Work with the Health Sub -Committee for the community activities. <br />9. Make a good faith effort to sustain and maintain the maps through other sources of <br />funding after the SHIP funding expires. <br />Deliverables <br />By June 24, 2010 the grantee will complete the following: <br />1. Develop a City of Hugo Trail System map will be produced for distribution to the <br />general public. <br />2. Write a plan for a series of healthy community events focusing on physical activity. <br />3. Document the promotion of existing physical activity policies. <br />4. Prepare and submit a SHIP evaluation report form to PHE. The report shall cover the <br />time period from January 3 through June 24, 2011. <br />5. Develop a sustainability plan beyond the grant funded period. <br />Budget <br />The total SHIP funding amount for this grantee will not exceed $4,000. The first half of the <br />funds will be released when a signed contract and invoice are returned. The second half of the <br />funds will be released when the list of deliverables is completed and the evaluation form is <br />returned, no later than June 24, 2011.
