Sandberg, P.E. <br />Deputy Director/County Engineer <br />WASHINGTON COUNTY ACCESS PERMIT #2016-Ar09 CHANGE IN USE OF EXISTING FIELD ENTRANCE TO A <br />RESIDENTIAL DRIVEWAY ON COUNTY STATE AID HIGHWAY 4 (170TH STREET) IN HUGO <br />Dear Dennis: <br />Washington County has approved the Access Permit for a residential driveway using the existing field entrance on <br />170th Street located approximately 700 feet west of Keystone Avenue. <br />The following permit conditions also apply to the permit: <br />• The access shall be located in the current location according to the drawing submitted. No changes or <br />alterations may be made at any time without written permission from Washington County. This permit is not a <br />guarantee or commitment that left turn movements or other aspects of egress and access allowed at the time <br />this permit is issued will continue in perpetuity. The county may, in the future as conditions warrant, prohibit, <br />regulate, or modify various traffic movements as to egress and access by means of signing, road modifications <br />or construction, or geometric changes within the road system. <br />• Culvert aprons shall be installed on the existing culvert. <br />• The County ditch slopes shall match existing slopes. <br />• The driveway approach grade shall match the cross slope of the roadway for a minimum distance of 15 feet. <br />• Where work on or near the roadway is necessary, proper signs, warning lights, shall be erected to protect <br />traffic and workers. All traffic control devices and methods shall conform to the Minnesota Manual on Uniform <br />Traffic Control Devices. This provision applies at all locations, including locations where vehicle parking is <br />allowed on the shoulder. <br />• All areas disturbed during construction in the right of way shall be restored and stabilized with proper erosion <br />control, including temporary erosion control as needed, within 7 days and maintained until the new turf is <br />established. <br />• No additional ponding shall occur in the County right-of-way. <br />• No foreign material such as dirt, gravel, or bituminous material shall be left or deposited on the roadway <br />during the work.
