d. The City Police Department agrees to provide victims of crimes diverted by local law <br />enforcement the opportunity for direct or indirect involvement into how the juvenile <br />should be held accountable, including consideration of the impact of the crime on the <br />victim and any specific needs for restitution. When necessary, due to the offense, <br />restitution will be determined and collected. <br />e. As appropriate, facilitate restorative justice interventions, such as victim/offender <br />mediation, family group conferencing, and/or community panels, so that juvenile <br />offenders are held direct accountable to the victim and victimized community through <br />some form of reparation. <br />f. Provide support to parents of youth offenders, and provide opportunities for youth to <br />access mentoring, enrichment, and leadership activities to keep youth from entering <br />the juvenile justice system. <br />2. Attachments and References — The Contractor agrees to provide the enumerated <br />services in Exhibit A, which is attached and incorporated herein by reference. <br />3. Cost and Delivery of Purchased Services — The total amount to be paid to the Contractor <br />for services purchased under this agreement shall be $6,000. For these services, the <br />City of Hugo agrees to make a single annual payment to the Contractor of $6,000 no <br />later than August 1, 2019. <br />4. Eligibility for Services —A City of Hugo resident between the ages of five and eighteen <br />(or older if still in high school), their family members, and the victims of juvenile crime <br />who are assessed to be appropriate shall be eligible for the services provided under the <br />terms of this agreement. <br />5. Evaluation Reporting, and Information Requirements — The Contractor agrees to <br />reasonable evaluations of its programs, employees, and volunteers and make them <br />available for review by the City of Hugo if so requested. The Contractor agrees to <br />comply with all reporting requirements as assigned by law, rule, or contract by the State <br />of Minnesota and Washington County. The Contractor further agrees to abide by all <br />laws and rules regarding confidentiality and data practices. The Contractor agrees to <br />provide necessary information allowed by law and deemed necessary by the City of <br />Hugo on referred cases. <br />6. Indemnification — The Contractor agrees to indemnify, defend, and hold harmless the <br />City, its officers, employees, and agents for all claims arising out of the Contractor's <br />activities related to the services provided under this agreement up to the liability limits <br />set forth in Minn. Stat. 466.04. The City of Hugo will indemnify Lakes Center for Youth & <br />Families, Inc. from and against all liability up to the liability limits set forth in Minn. Stat. <br />466.04, No other provision of this agreement shall serve to limit in any way the <br />obligations of the Contractor to indemnify and defend the City of Hugo under this clause. <br />7. Insurance Requirements — The Contractor agrees that in order to protect it self, as well <br />as the City of Hugo, from claims arising out of the Contractor's activities under this <br />agreement, it will at all times during the term of this agreement keep in force policies of <br />insurance providing the following liabilities: professional liability insurance policy in the <br />minimum amount of Four Hundred Thousand Dollars ($400,000) bodily injury or death of <br />one person; One Million Two Hundred Thousand Dollars ($1,200,000) for bodily injury <br />
