RESOLUTION 2021-64 <br />APPROVING A SITE PLAN FOR LAKEVIEW FAMILY DENTISTRY FOR <br />SITE PLAN APPROVAL FOR PROPERTY LOCATED AT 14475 FOREST <br />BOULEVARD NORTH <br />WHEREAS, Lakeview Family Dentistry has requested site plan approval for an <br />approximately 4,400 square foot addition to be constructed on to the existing building on <br />property located at 14475 Forest Boulevard North that is legally described as follows: <br />See attached. <br />WHEREAS, the Planning Commission has reviewed said site plan on September 23, <br />2021, and recommends approval subject to the conditions listed in this resolution. <br />NOW, THEREFORE, BE IT HEREBY RESOLVED BY THE CITY COUNCIL OF <br />THE CITY OF HUGO, MINNESOTA, that it should and hereby does approve the site <br />plan for Lakeview Family Dentistry with the following findings: <br />1. A site plan is approved to allow construction of an approximately 4,400 square foot <br />square foot addition with the plans dated September 16, 2021. <br />2. The applicant shall obtain all necessary local, state, and federal permits. <br />3. The applicant shall provide an executed temporary construction agreement for work <br />being done on the adjacent property to the north and shall provide a concurrent <br />grading plan to be reviewed and approved by staff. <br />Prior to Building Permit Approval: <br />4. All items on the Engineer's memo and "red -line" detailed engineering comments that <br />have been sent directly to the applicant's engineer dated September 20, 2021, shall be <br />addressed. <br />5. The applicant shall submit the final building material sample board to be reviewed <br />and approved by staff. <br />6. The final location of the trash enclosure shall be reviewed and approved by staff. <br />Prior to issuance of a Certificate of Occupancy: <br />7. A final landscaping and lighting plan shall be reviewed and approved by staff and all <br />required landscaping shall be fully installed. <br />8. Drainage and utility easements shall be dedicated to the City over the stormwater <br />management facilities and applicable utilities. <br />
