Page 3 of 3 The undersigned agrees to the following terms and conditions, along with the Public Facility Policy and all other city regulations that govern the use of Hugo’s public facilities. o The undersigned acknowledges that the facility is in good condition and that it is safe for organization or group to use. o The undersigned will be responsible for all loss, damage, or claims made against them or their party arising out or related to their group’s use of the facility, including damage to facility, loss of equipment, and costs of collection including reasonable attorney fees. o The undersigned covenants and agrees to hold the City harmless and to indemnify the City against any and all claims and liabilities for compensation. o The undersigned understands that all organization members can be held legally liable for improper use of the facility. o The undersigned understands that their signature is made on behalf of their organization. The City may request additional group signatures, or proof of facility rental authorization within the group. o The undersigned understands that the use of the facility is subject to the timely payment of all required fees and deposits, as adopted by the City. o The undersigned understands that this application is subject to approval by the city. If approved, this application becomes a binding agreement between the undersigned and the City of Hugo. o The undersigned certifies that information provided herein is true and correct.
