<br />: <br /> Page 54 <br />Section 21.03 Unsafe Behavior <br /> <br />Supervisors are authorized to send an employee home immediately when the employee’s <br />behavior violates the City’s personnel policies, department policies, or creates a potential health <br />or safety issue for the employee or others. <br /> <br />ARTICLE 22 MISCELLANEOUS POLICIES <br /> <br />Section 22.01 Personnel Records <br /> <br />The City Administrator is responsible for establishing and maintaining an official personnel file <br />for each employee of the City. The Fire Chief shall be responsible for maintaining personnel files <br />for all firefighters, with the exception of any medical information, which shall be maintained by <br />the City Administrator. <br /> <br />Official Personnel files maintained by the City Administrators Office will include all material of a <br />confidential nature to include, but not limited to: <br /> <br />• Psychological Profiles <br />• Job qualification Tests <br />• Polygraph Results <br />• Background Checks <br />• Oral Board Results <br />• Performance Plans and Reviews <br />• Employment Contracts <br /> <br />Access to the Personnel files may be allowed if the City Administrator or Fire Chief feels access <br />would be helpful, necessary or warranted for administrative purposes. <br /> <br />Department Heads are responsible for the forwarding of documents for inclusion in the <br />Personnel files of those employees assigned to their department. <br /> <br />Each employee is responsible for the verification of information contained in the personnel file <br />through periodic audit. An administration representative must be present when the file is <br />audited. Only the City Administrator may remove items from the respective files with notification <br />to respective Department Head. The Department Head shall schedule all audits or inspection of <br />records by employees. All audit appointments should be with the City Administrator. The <br />purpose of the audit or inspection is to ensure accuracy and completeness of the file. <br /> <br />Separate personnel files shall not be maintained by individual departments, with the exception <br />of the Fire Department. <br />
