SECTION 8. CERTIFICATION, LICENSES AND PERMITS <br /> 801 . Certification . <br /> 801 .01 . Effective April 1 , 1989, any person , firm or corporation <br /> proposing to engage in the business of soil testing , design , <br /> construction , installation ., or pumping of on-site sewage treatment <br /> systems within the jurisdiction of this Ordinance, shall be required <br /> to provide to the Zoning Administrator, proof of certification by the <br /> MPCA. <br /> 8C)2. Licensing . <br /> 802.01 . All persons, firms, or corporations proposing to engage <br /> in the business of installation , constructing, pumping , or providing <br /> maintenance services for Individual Sewage Treatment Systems within <br /> the City shall first obtain an annual license to carry on such work . <br /> 802.02. Annual licenses shall be in effect from January 1 to <br /> December 31 and shall be valid only during the year for which issued. <br /> 802.03. License applications shall be submitted to the Zoning <br /> Administrator and shall contain the following: <br /> ( 1 ) Name of individual , firm, or corporation seeking <br /> license. <br /> { (2) After April 1 , 1989, proof of certification by the <br /> MPCA. <br /> (3) Documentation indicating insurance coverage which shall <br /> remain in effect during the license term and <br /> noncancel lation provisions which provide a minimum ten <br /> ( 10) day notice to the Zoning Administrator prior to <br /> cancellation , as follows: <br /> a. Public liability insurance, not less than one <br /> hundred thousand dollars ($100,000.00) for <br /> inJuries including accidental death to any one ( 1 ) <br /> person and subject to the same limit for- each <br /> person in an amount of not less than three hundred <br /> thousand dollars ($300,000.00) on account of any <br /> one ( 1 ) accident. <br /> b. Property damage insurance, not less than fifty <br /> thousand dollars ($50,000.00) for each accident <br /> and not less than one hundred thousand <br /> ($100,000.00) aggregated . <br /> c . Workmen ' s compensation insurance coverage of <br /> employees as required by Minnesota Statutes. <br /> 44 <br />
