RESOLUTION 2015-51 <br /> APPROVING SITE PLAN FOR DIGNICARE,LLC.ON PROPERTY LOCATED <br /> SOUTH OF 130TH ST.AND EAST OF FOREST BLVD,(TH 61) <br /> WHEREAS, DigniCare, LLC.,has requested approval of a site plan to allow construction <br /> on the 3.34 acre property legally described as follows: <br /> (See Attached) <br /> WHEREAS, the Planning Commission has reviewed the site plan at a duly called Public <br /> Hearing and recommends approval,and; <br /> NOW, THEREFORE, BE IT HEREBY RESOLVED BY THE CITY COUNCIL OF <br /> THE CITY OF HUGO, MINNESOTA, that it should and hereby does approve the <br /> request by DigniCare, LLC,for a site plan,subject to the following conditions: <br /> 1. A site plan is approved in accordance with the site plan dated September 18,2015 and <br /> October 1,2015, except as amended by this resolution. <br /> 2. Approval is contingent upon City Council approval of a rezoning to PUD for the 3.34 <br /> acre site. <br /> 3. The project is approved for a maximum of 32 senior housing units. <br /> 4. A 39 foot wide roadway easement shall be dedicated for the expansion of Forest <br /> Blvd. (TH 61). <br /> 5. Prior to the issuance of a building permit, the developer shall submit final site plans, <br /> building elevations, street improvement plans, utility plans, grading and drainage <br /> plans, landscaping plans, lighting and signage plans along with the required land use <br /> applications for the development. <br /> 6. Prior to the issuance of a building permit, the developer must comply with the <br /> following conditions: <br /> a. The developer must comply with Resolution 2015-50 for the PUD. <br /> b. The developer must obtain all necessary stormwater management and WCA <br /> permits from the City. <br /> c. The developer shall obtain all necessary local, state,and federal permits. <br /> 7. Prior to the issuance of a building permit, the developer must comply with the <br /> following conditions: <br />
