APPROVING A SITE PLAN FOR NATIONAL SENIOR HOUSING SERVICES,
LLC, FOR A SENIOR HOUSING FACILITY ON THE PROPERTY LOCATED
NORTH OF 145TH STREET AND WEST OF FINALE AVENUE NORTH
WHEREAS, National Senior Housing Services, LLC, requested approval of a site plan
for a senior housing facility on the property legally described as;
Lot 1, Block 1, Lavalle Fields
WHEREAS, the Planning Commission has reviewed said site plan on April 24, 2014, and
recommends approval subject to the conditions listed in this resolution.
NOW, THEREFORE, BE IT HEREBY RESOLVED BY THE CITY COUNCIL OF
THE CITY OF HUGO, MINNESOTA, that it should and hereby does approve the site
plan for National Senior Housing Services, LLC, for a senior housing facility with the
following findings:
1. Approval is contingent upon City Council approval of the Final Plat for Lavalle
Fields.
2. The applicant shall meet all requirements in Resolution 2013- 3 and 2013-5 for
Lavalle Fields Preliminary Plat and Planned Unit Development (PUD).
3. All items on the Engineer's memo and "red -line" detailed engineering comments that
have been sent directly to the applicant's engineer dated April 18, 2014, shall be met.
4. The development shall be consistent with the plans submitted on April 11, 2014 and
April 14, 2014.
5. The applicant will be responsible for maintaining stormwater facilities and vegetation
proposed as a part of the project. A maintenance agreement between the City and the
applicant shall be reviewed and approved by the City then recorded against the
property prior to the start of grading activities on the site.
6. The irrigation system shall be designed and installed for it to be connected to the
large stormwater pond planned north of the property as part of phase 2 of the Lavalle
Fields PUD. At the time the stormwater pond is constructed, the system shall be
converted to irrigate the property with stormwater.
7. The applicant shall be required to apply for formal site plan approval for all future
phases to be considered by the Planning Commission and City Council.
8. The monument sign location shall be removed from the public right of way. The
monument sign shall be designed using materials consistent with the building
materials.

9. Prior to the installation of any signs, the applicant shall obtain a sign permit.
10. The portion of the trail used for the fire lane shall be plowed during the winter to
allow access.
11. Any areas of the trail along CSAH 8 that are disturbed from the construction access
into the site shall be restored to the City's satisfaction.
Prior to the issuance of a building permit:
12. The Final Plat and Development Agreement for Lavalle Fields shall be recorded.
13. The landscape plan shall be revised based on staffs comments. The landscape plan
shall be reviewed and approved by staff.
14. The applicant shall submit final design cut sheets for the pole and wall mounted light
fixtures to be reviewed and approved by staff.
15. The final design of the canopy shall be reviewed and approved by staff.
16. The applicant shall revise the architecture on the west elevation above the
underground parking area to include elements, such as larger windows, overhang
details, and revise the placement of the stone, to match the other elevations of the
building.
17. The gutter and downspout system shall be integrated into the building design and
shall be reviewed and approved by staff.
18. Construction details of the fire lane for the senior housing facility shall be reviewed
and approved by staff.
19. The location of the roof top and ground mechanical equipment shall be reviewed and
approved by staff.
Prior to issuance of a Certificate of Occupancy:
20. All roof top and ground mechanical equipment must be screened from view of
adjacent properties. Staff shall review and approve the locations. All ground -level
mechanical units shall be low profile and located to the side or rear of the building
and shall be screened from view of adjacent properties. Staff shall perform an
inspection when equipment is installed to ensure compliance with City code.
21. Finale Avenue between 145th Street and 146th Street shall be reconstructed as an
urban section roadway to a width of 26 feet to provide adequate street conditions for
the access into the senior housing facility. The construction plans will be reviewed
and approved by staff.

22. A 10 foot wide public trail easement shall be dedicated to the City for the trail shown
on the site plan.
23. The trail shall be constructed as shown on the site plan.
24. All required landscaping shall be fully installed.
25. Drainage and utility easements shall be dedicated to the City over the stormwater
management and sewer extensions.
