RESOLUTION NO. 2006-33
A RESOLUTION THE DISSOLUTION OF THE MEMORIAL HOSPITAL
DISTRICT AT FOREST LAKE
WHEREAS, the City of Hugo is the city included in the Memorial Hospital
District at Forest Lake (the "Hospital District'), a hospital district created pursuant to
Chapter 447 of the Minnesota Statutes; and
WHEREAS, the Hospital District conveyed the facility in Forest Lake, Minnesota
to Fairview Health Services in March 2006; and
WHEREAS, it appears that the conditions for the creation of the Hospital District
no longer exist within the meaning of Chapter 447 of the Minnesota Statutes; and
WHEREAS, Minnesota Statute § 447.38 provides that a city that is included in a
hospital district may petition the governing board of the hospital district to dissolve
pursuant to the provisions of Chapter 447 of the Minnesota Statutes; and
WHEREAS, the staff has presented the City Council with the form of a Petition
for Dissolution of Hospital, a copy of which is attached to this Resolution; and
WHEREAS, the Petition for Dissolution of Hospital is in a form and substance
acceptable to the City Council.
NOW, THEREFORE, BE rr RESOLVED,
1. The Mayor is hereby authorized and directed to execute and deliver the attached
Petition for Dissolution of Hospital to the Governing Board of the Hospital
District.

PETITION FOR DISSOLUTION OF HOSPITAL DISTRICT
The City of Hugo, Minnesota, pursuant to Chapter 447 of the Minnesota Statutes,
hereby petitions the governing board (the "Governing Board") of Memorial Hospital
District at Forest Lake (the "Hospital District") as follows:
1. The Governing Board is hereby requested to undertake and complete all
action necessary and appropriate to dissolve the Hospital District pursuant
to, and in accordance with, Chapter 447 of the Minnesota Statutes.
2. The grounds for this Petition is that the facility Forest Lake, Minnesota,
previously owned by the Hospital District, has been conveyed and the
remaining assets of the Hospital District consist essentially of funds in
deposit in bank accounts, and it appears that the conditions for the creation
of the Hospital District no longer exist.
3. The City of Hugo proposes the following for the distribution of the
remaining funds of the Hospital District among the cities and other
governmental subdivisions:
Divide the assets equally among the five constituent municipalities.
4. This Petition is submitted pursuant to Resolution No. 2006-33 of the Hugo
City Council dated June 19, 2006.
