Service Description <br />Extension of services and support of the City of Roseville 's Cisco Internetworking Protocol (IP) <br />Telephony System for Falcon Heights City Hall. <br />C. Scope <br />This service agreement provides for the technical support of Cisco IP Telephony devices purchased and owned by <br />the City of Falcon Heights use in conjunction with the telephony services equipment owned and operated by the City <br />of Roseville. The agreement does not cover arty non-Cisco telephony devices that may be owned by the City of <br />Falcon Heights. This service agreement also does not cover the use of the IP telephony devices with any other <br />telephony servers or service providers other than that of the City of Roseville. <br />The service agreement shall cover hardware and software support of IP telephony devices purchased during the <br />• initial project. This consists of 20 IP telephony handsets and 4 analog service ports (Cisco ATA devices). The <br />agreement shall also cover additions not to exceed 25% of the initial purchase. This provides for up to 4 additional <br />devices without modification to this statement of service. <br />As part of this service agreement, Falcon Heights will be allocated 2 blocks of 20 direct inward dial (DID) <br />telephone numbers. This will provide 40 individual extension numbers for use by Falcon Heights. <br />D. Terms <br />Support services shall be billed annually at a flat rate of $2, 500, effective the first day of service of the telephony <br />contract. Installation and programming services will be covered by the existing technology services joint powers <br />agreement. <br />This support service agreement does not include monthly telephony and long distance use charges which are billed <br />monthly at a rate of $1 S per month device charge and $6 per month for each voice mail box. This rate shall remain <br />in effect during the first 12 months of service. <br />Long distance phone service is through the City ofRoseville's current contract with MCI which provides a rate of 5- <br />13 cents per minute based on calling area and time of day. A long distance call detail report is provided as part of <br />the monthly services billing. <br />D, Termination <br />Upon termination of this service addendum, hardware devices purchased by the City of Falcon Heights will remain <br />property of the City of Falcon Heights. Use of the Roseville IP telephony system does not provide the transfer of any <br />software licenses used to connect telephony devices to the network. Contiguous blocks of allocated DID telephone <br />numbers, if requested, will be transferred to the City of Falcon Heights through Qwest to be used on another <br />• telephone system. <br />3S <br />
