5. A description of procedures to be followed for handling <br /> missed pickups or customer complaints. At a minimum, the <br /> following should be addressed: <br /> a. A phone number available until 6:30 p.m. on pickup day. <br /> b. A procedure for pickup that evening or the following <br /> day before 3:00 p.m. <br /> c. Availability of vehicles to handle missed pickups. <br /> 6. A description of how collection tonnages will be verified <br /> and how records will be kept for reimbursement. This <br /> information should include: <br /> a. Original truck scale tickets; <br /> b. Name of collection contractor and market, and the type <br /> and weight of each recycled product collected (see <br /> list of required products); <br /> c. Sample of contractor's current report form. Tonnages <br /> shall be reported in pounds collected. Totals for <br /> multi -unit complexes of 18 or more units must be <br /> reported separately from totals for single family <br /> residences and multi -unit complexes of 17 or fewer <br /> units. <br /> 7. A statement that all trucks and equipment will be <br /> maintained in good working condition, be marked with the <br /> contractor's name, and be constructed in such a way as to <br /> contain all materials from spilling, leaking, or blowing <br /> out of the vehicle. <br /> 8. A description of any innovative provisions in the proposal, <br /> such as profit- sharing or other incentive programs, <br /> collection of materials other than those required, or any <br /> other provisions which would make the bid unique. <br /> 9. Each contractor shall provide a bond equal to three months <br /> hauling fees, collectable upon contract cancellation, poor <br /> performance or contract violation. The bond and proof of <br /> insurance will be required at the time the contract is <br /> signed. <br /> 10. The contractor must give a description of all proposed <br /> depositories for recyclable materials. <br /> 11. The contractor must provide a description of previous <br /> experience in collecting, processing, and marketing <br /> recyclable materials. <br /> 12. The contractor must provide a written policy regarding the <br /> handling of customer complaints and general customer <br /> service. <br /> -2- <br />
