SPECIFICATIONS FOR RECYCLING REOUEST FOR PROPOSAL <br /> All proposals must be received by =4-30 p.m., December 18, 1992, at <br /> City Hall, 2077 West Larpenteur Avenue, Falcon Heights, Minnesota <br /> 55113. <br /> All Proposals Must Contain the Following Items: <br /> 1. Name, address, telephone number and contact person. <br /> 2. Rates, including details of any alternates bid (see page 3). <br /> a. Single family residences shall be bid on a per <br /> household basis. <br /> b. Multi -unit complexes of 17 or fewer units shall be bid <br /> on a per unit basis. <br /> c. Multi -unit complexes of 18 or more units (of which <br /> there are seven) shall be bid on a flat charge basis. <br /> 3. Completed city business license application. Attach <br /> current financial statement to license application. <br /> 4. Description of how service will be provided, including: <br /> a. Number of collections per household per month for <br /> single family residences and multi -unit complexes of <br /> 17 or fewer units. (Proposed rates for alley and <br /> curbside service to these households should include <br /> both first and third Friday collection and weekly <br /> collection.) <br /> b. For multi -unit complexes of 18 or more units, which <br /> shall receive weekly collection, the method of <br /> determining cost of collection. <br /> c. A description of specific equipment to be used. <br /> Provide pictures and axle weights of trucks, <br /> intermediate processing vehicles and equipment. <br /> d. A list of products to be collected. Collection of the <br /> following products is required: newspaper, glass <br /> containers, aluminum and steel cans, corrugated <br /> cardboard, plastic, magazines, catalogs and glossy <br /> paper, used motor oil, automotive batteries, and <br /> office paper. <br /> e. Material preparation instructions that will be given <br /> to residents. <br /> -1- <br />
