<br /> <br />Page 2 of 8 <br />Adaptive Recreation and/or Inclusion Services Contract Agreement 2025 <br /> <br />Reach for Resources, Inc. and the City of Little Canada (Minnesota) agree that Reach for Resources Inc. will <br />provide Inclusion services and training for Little Canada residents and programs. The contract fee is $3,024.10 to be paid <br />annually upon receipt of an invoice. The services will begin on January 1, 2025 and continue until December 31, 2025. <br /> <br />General Benefits of Membership - $1,831.98 <br /> Membership in the Reach consortium allows for utilization of Reach’s recreation team to coordinate and <br />facilitate inclusion opportunities (depending on the package(s) purchased). <br /> After becoming a member, partners may purchase their choice of inclusion packages to support their residents <br />with disabilities. <br /> All Reach staff will be trained and insured under Reach for Resources. Reach contracts ensure city partners are <br />meeting ADA regulations for inclusive services in a cost-effective way that allows for high quality services to <br />residents with disabilities. <br /> Membership gives the opportunity to purchase inclusive recreation program services at below market, nonprofit <br />prices. <br /> Members have access to Reach’s professional staff by phone for information and referral, consultation and <br />assessment about disability related recreational concerns and needs. <br /> Members have access to Reach’s professional staff in person on a biannual basis to discuss access issues, ADA <br />law, and exchange ideas. <br /> Members can purchase professional, disability-related training sessions at below market, nonprofit rates. These <br />are an optional contract add-on. Training options can be viewed and requested here: <br />https://forms.office.com/r/GXwbr1s6n6 <br />o 60-90 minute in-person training of your choice in 2025 is $311.44. This also includes the video link for <br />any number of learners. <br />o 60-90 minute recorded virtual training in 2025 is $123.81. <br /> Little Canada staff will have access to Reach’s certified, adaptive recreation professionals, licensed social <br />workers, and special education staff by phone for information, referral and consultation about disability related <br />recreational concerns and needs for up to the number of administrative hours purchased (28 hours). <br />o If Little Canada utilizes more than 28 administrative hours, additional hours will be billed at $46 per hour <br />in 15 minute units. <br /> Reach will coordinate and facilitate semi-annual meetings including Reach’s professional staff and other service <br />recipients to problem solve, exchange ideas, review services provided, delivery methods devised, and results <br />achieved in the prior 6-month period. <br /> Membership gives Little Canada access to the Reach inclusion questionnaire and other service-related forms <br />upon request. <br /> <br /> <br />
