<br /> <br />Page 3 of 8 <br /> <br />Inclusion Package - $880.68 <br /> <br />Inclusion services ensure programs are ADA compliant and that individuals of all abilities are welcomed into <br />programming in a way that sets up the program and the individual up for success. Inclusion service provided by <br />Reach shall include both the professional and paraprofessional services below: <br /> <br />Reach for Resources Professional Services (28 Hours for Little Canada) <br />All of these services listed (except those with an *) below will be tracked as administrative time when they are done for a <br />specified partner. General hiring, training and supervision of Reach staff will not be tracked unless it relates to a specific <br />request. <br /> Consultation with staff about general programmatic or specific resident’s needs; <br /> Observation and needs assessment of individuals with disabilities; <br /> Inclusion plan development and virtual inclusion intake meetings; <br /> *Recruitment and training of inclusion facilitators on an ongoing basis; <br /> Individualized coaching for specific recreation generalists who are including individuals with disabilities; <br /> *Ongoing supervision and management of Reach’s paraprofessional inclusion staff; <br /> *Reports compiling inclusion hours provided semi-annually; <br /> More detailed or frequent reporting upon request; <br /> Inclusion Progress reports available upon request; <br /> Ongoing communication between family, Reach professionals and program leaders to ensure coverage and <br />preparedness <br /> *Little Canada will receive completed inclusion questionnaires upon request, only to be shared on a need-to- <br />know basis with team members directly supporting the participant. <br /> <br />Paraprofessional Services <br /> Implement the inclusion plan <br /> Attend scheduled programs and provide direct assistance to the identified individual(s) <br /> <br />Inclusion Policies and Procedures <br /> Reach inclusion services are provided only to programs planned, run, and registered for within the contracted <br />entity. Reach cannot provide inclusion services for partnered programs unless the registration is completed <br />through the contracted entity. For further assistance, program partners may contact Reach for contract options. <br /> Reach recreation staff are paid a minimum of two hours for each shift, thus Reach will bill a minimum of two <br />hours for each scheduled inclusion unless otherwise noted above.
