MEMORANDUM <br />TO: Demolition Contractors <br />FROM: Joel Hanson, City Administrator <br />DATE: March 2, 2012 <br />RE: Request for Quotations <br />The City of Little Canada is seeking a quotation for the removal of a fire damaged garage and <br />associated debris (see photos attached). The property is located at 2600 Spruce Street. The City <br />has not done a hazardous material evaluation and the selected contractor will be responsible for <br />ensuring compliance with all laws associated with the disposal of this debris. <br />We are seeking quotations based upon the following criteria: <br />• All abatement activities (removal, storage, transportation, and disposal) performed <br />under this project must be done in conformance with all laws and regulations of the <br />Little Canada, Ramsey County, the State of Minnesota as well as the United States <br />Environmental Protection Agency. The selected contractor is responsible for <br />providing all notices to the appropriate governmental authorities having <br />regulatory oversight over the abatement /demolition activities. <br />• Each contractor submitting a quotation shall include the appropriate <br />certifications /licenses evidencing that they are legally qualified to undertake the <br />work in question. <br />• The contractor awarded this job must provide workers compensation insurance at the <br />mandated statutory limits. The contractor shall also provide comprehensive general <br />liability coverage with minimum limits of $1,000,000 per occurrence. The City of <br />Little Canada shall be named as an additional insured and be provided with a <br />certificate of insurance with a minimum of ten (10) days notice of cancellation. <br />• Any applicable utilities will be disconnected by others prior to the commencement of <br />demolition activities. <br />• The successful contractor shall defend, indemnify, and hold harmless the City of <br />Little Canada, its employees, elected officials, agents, and assigns from any act <br />associated with the contractors performance of this work. <br />• The quotation shall also include any needed clearance testing required by regulatory <br />agencies in order to ensure the demolition debris generated under the subsequent <br />building demolition contract do not have to be treated as contaminated or hazardous <br />materials. <br />• All work associated with this project must be completed by May 31, 2012. No work <br />may be undertaken on the site if compliance with the 5 -ton road restriction on Spruce <br />Street cannot he achieved. Failure to complete the required work by the stated date <br />shall result in the imposition of liquidated damages of $400 per day for each calendar <br />day of delay. <br />• Payment shall be made in a lump sum within thirty (30) days of completion of the <br />work. The Contractor shall provide a copy of all records and reports filed with the <br />State of Minnesota relative to this project evidencing completion pursuant to <br />2 <br />
