..C,1HMONG MINNESOTA <br />U SENIOR CENTER INC. <br />1730 Gervais Avenue Maplewood, MN 55109 <br />March 23, 2012 <br />Objectives <br />To give a physical and mental healthy environment so we can delay Hmong elders <br />from going to nursing homes. <br />Mission <br />• To provide an additional quality care option in addition to at home personal <br />care attendance (PCA). <br />• To keep Hmong elders in connection with their community to ensure their <br />quality of life, their sense of self -worth and dignity as remained. <br />Keys to Success <br />® Ying Chu - Yang -Heu has 10 years of experience with elderly management, <br />recruiting, connection with the State of Minnesota and the 7 counties in the <br />metro area. <br />® Have been a successful business for the last 5 years. <br />® Good, healthy and caring environment for the elders. <br />• Good communication toward clients, their families, their counties and the <br />state. <br />o Trust and acceptance of the center from the elders, so it can grow to its full <br />capacity with word -of- mouth. <br />Executive Summary <br />The Twin Cities area has a Hmong population of 65,000 and growing, with about 2 % <br />of that group being over 50 years old with disability and need of quality care. The <br />Hmong Minnesota Senior Center (HMNSC) will seek to fulfill that need by providing <br />care to Hmong elders in the metropolitan residential area. Senior care is extremely <br />important and very demanding, especially due to the cultural sensitivity required in <br />this case. It is to serve this specific population that we want to start this senior care <br />center, With the expertise of our staff who speaks Hmong, French, English, Thai and <br />Lao, we know we can provide the care necessary. <br />
