e Page 2 March 23, 2012 <br />Company Summary <br />Hmong Minnesota Senior Center will provide senior care to an average of 80 seniors <br />on a daily basis. We have a knowledgeable staff that has many years of experience <br />in the industry. We also understand the needs of this specific group and have <br />relationships within the community, making finding prospective clients easier. All <br />HMNSC activities are conducted inside the building, there are no activities outside <br />the building. The only traffic would be when vans head out to pick -up and send <br />clients home. <br />Market Analysis Summary <br />According to the Metropolitan Area Agency on Aging, in the year 2010, in the 7 metro <br />area counties there was a total of 2,957 Hmong over the age of 50 with disability and <br />need of quality care. They break down as follow: <br />® 1,891 elders between the age of 50 and 64 <br />® 584 between the age of 65 and 74 <br />® 365 between the age of 75 and 84 <br />® 117 over 85 of age <br />In comparison, the metropolitan area only has seven Hmong elderly senior centers <br />that are providing services to Hmong elders, each of them operating at an average <br />maximum capacity of 50 clients which is a total of 350 clients being served and more <br />than 2600 without services. <br />Strategy and Implementation Summary <br />We expect to pick up elders at their homes every morning around 8 AM and drop <br />them back around 4 PM (see Schedule 1 for daily function). We will provide nutrition <br />breakfast, lunch and snack at the center. Our main focus is health and social <br />rehabilitation. To implement that, we will have daily exercises such as card games, <br />bingo, arts and crafts etc... so the elders can have some mind exercises, We will <br />also have physical exercises such as light stretch exercises, light walks and exercise <br />machines to help the elders with their physical health. We will also have social time <br />for the elders, so they can have a chance to make friends and slowly rehabilitate <br />them - selves into a social environment. We will also have daily prizes for winners of <br />games during the day. We will also have special field trips to visit certain things that <br />the elders have never seen before, such as; State Capital, museums, national parks <br />etc... <br />
