This agreement allows us to direct your <br />payment to the actual Mn /DOT office that provides the service to you. By specifically <br />directing your payment to the appropriate office, we are able to more adequately fund that <br />office and the services they provide, and to more confidently ensure the services will be <br />retained in the future. <br />Attached is a proposed Blanket Partnership Agreement for your consideration. <br />This agreement should be used only for technical services that are small, routine and <br />under $10,000. Please refer to the list of services shown on Exhibit A of the Agreement. <br />Large, complex, or unique partnerships will continue to be developed individually with the <br />appropriate legal and policy review. <br />Please follow these steps for executing this agreement: <br />1. Have 2 officials sign all three originals of the agreement. <br />2. Attach an original copy of your County Board or City Council resolution OR a note <br />from they County /City Attorney that a resolution is not needed. The resolution must <br />indicate that the proper county /city officials have executed the agreement. <br />3. When the State has fully executed the agreement, you will receive one original for <br />your files. <br />When the agreement is fully executed, and you want to request a service: <br />1. Contact me at 651- 582 -1609. I will direct your request to a Mn /DOT project manager. <br />Exception: Continue to directly contact the Mn /DOT Lab for material testing and tell <br />them you have a Blanket Partnership Agreement on file with Metro State Aid. <br />2. Identify a project manager from your jurisdiction. Include his /her name, phone, <br />address, and email with your request. <br />2 <br />
