Fahey <br />COUNCIL <br />Jim LaValle <br />Matt Anderson <br />Rick Montour <br />Bill Blesener <br />ADMINISTRATOR <br />iTOR <br />Joel R. Hanson <br />Life insurance is provided for regular full -time employees by the City. Regular € ull -time <br />employees - are -enc urabed to-purchase standard life - insurance coverage -as- provided -by <br />the City's insurance carrier.- Regular full -time employees have the option of purchasing <br />additional coverage over the amount provided by the City as well as have the option of <br />purchasing, dependent coverage. For those employees purchasing additional coverage <br />and /or dependent coverage, the The -cost of this coverage is deducted from paychecks on <br />a bi- weekly basis_ with -the City contributing-$9.50 toward the c st of this - insurance. <br />Additional- eoverage limits can -be- purchased by the- empleyee at his/her - discretion. <br />Opt -Out Program <br />Employees who are eligible for participation in the City's Health Insurance Program are <br />eligible for the Opt -Out choice. The City offers Opt -Out in order to recognize the needs <br />of our diverse workforce by providing maximum flexibility in health care choices <br />available to employees and their families. Employees are responsible for evaluating the <br />feasibility of this option in terms of their family's health insurance needs. <br />This alternative allows employees who are covered under another medical plan to "opt - <br />out" of coverage and receive a cash payment in lieu of medical benefits. Employees <br />choosing Opt -Out will receive half f-what would -have been the- Gity's- c-estfhealth <br />insurance -for the - employee- and/ereligible- depend ts, if applicable, the equivalent of the <br />
