an illness. No employee who has been asked to provide such a <br />statement shall be allowed to return to work until they comply with <br />this provision. <br />Medical Appointments. Employees are encouraged to arrange for <br />doctor, dentist and other medical appointments before or after your <br />workday. All appointments during scheduled work hours must <br />have prior approval by your Supervisor. Emergency time off will <br />be handled on an individual basis. <br />Employees may use two (2) days of their annual sick leave in 15 <br />minute increments for doctor -er dentist appointments for their own <br />medical and dental health care as well as for thou children. <br />Documentation of time used shall be furnished to the employee's <br />supervisor on the approved form. <br />If an employee has no (zero) sick leave usage within a twelve- <br />month period commencing July 15` and ending the following June 30`h, the <br />employee may then covert four sick leave days to two vacation days at the <br />conclusion of that one year period. <br />Page 16 <br />
