Excavation Permit Fee: $ [To be flied out by the City.] <br />a: City Cost: $ <br />b: Cost of Restoration (if done by the City) $ <br />c: Degradation Cost: $ <br />(In an amount to fully cover costs associated with the decrease in the useful life of the Right-of-Way <br />caused by an excavation.) <br />2. Obstruction Perfnit Fee: $ <br />(In an amount sufficient to cover the City's Cost.) <br />3. Disruption Fee: $ <br />(Note: The City will impose $ per day for delays in the completion of an Excavation or <br />Obstruction beyond five (5) days past the period Indicated in This Permit.) <br />Note I: "City Cost" means the actual costs incurred by the City for managing Rights - <br />of -Way including, but not limited to costs associated with registering of <br />applicants; issuing, processing, and verifying Right -of -Way Permit • <br />applications; revoking Right -of -Way Permits; inspecting job sites; creating and <br />updating mapping systems; determining the adequacy of Right -of -Way <br />restoration; restoring work inadequately performed; maintaining, supporting, <br />protecting, or moving user equipment during Right -of-Way work; budget <br />analysis; record keeping; legal assistance; systems analysis; and performing all <br />of the other tasks required by this Chapter, including other costs the City may <br />incur in managing the provisions of this Chapter except as expressly prohibited <br />by law. <br />Note 2: The Excavation or Obstruction Permit Pees must be paid at the time of <br />application or within thirty (30) days of billing therefore by the City.
