1.) <br />SPECIFICATIONS FOR DEMOLITION <br />The City of Little Canada requires the successful bidder to provide <br />a surety bond or cashier's check in the amount of $1,000.00 per <br />structure to ensure compliance with specifications. This will be <br />returned upon satisfactory completion of the project. Should work <br />not be completed to the satisfaction of the City, notice will be <br />given allowing for 10 days to correct deficiencies. Should they not <br />be satisfactorily addressed, the contractor will forfeit his /her <br />rights to the deposit. <br />2.) Successful bidder(s) will be required to provide a certificate of <br />insurance with minimum limits of $100,000 /person, <br />$300,000 /occurrence for general liability and property damage. <br />Evidence of worker's compensation insurance must also be provided. <br />Certificates must indicate insurance coverage in effect while work <br />is in process. The City shall be named as an additional insured. <br />3.) Quotations are to be valid until Friday, October 30, 1992. The <br />contractor will have 14 days to complete demolition and restoration <br />activities once work is commenced. In no case may the completion <br />date extend beyond 30 days from the date the contractor has been <br />notified of contract award. <br />4.) A demolition permit must be obtained from the City before work <br />commences. The cost is $25.00 per structure. <br />5.) The structure in question must be completely demolished to the <br />foundation. Any part of the building or items within the building <br />which are not part of the foundation are to be removed. The <br />foundation is not to be damaged. <br />6.) Successful bidder(s) shall provide safe measures of protection at <br />the work site during the demolition process. Security fence shall <br />be placed around open excavations, windows and doors. <br />7.) Successful bidder(s) must ensure that the Gopher State One Call has <br />been called for the location of all utilities. Protection of said <br />utilities is the responsibility of the Contractor. <br />8.) If the windows or doors are removed prior to the actual demolition, <br />access must be eliminated by boarding or another comparable measure. <br />9.) Successful bidder(s) must ensure that water and sewer systems have <br />been abandoned in accordance with City and State regulations. <br />10.) Evidence must be provided that any well has been abandoned by a <br />State licensed Contractor. A deduct for well abandonment by others <br />must be included with this quotation. <br />11.) Concrete steps and slabs must be removed from the work -site and <br />disposed of in an approved manner. <br />12.) Assure that clean fill will be placed in excavated areas to match <br />surrounding grade and seeding of disturbed areas will be done within <br />the allotted time frame. <br />Page 3 <br />
