Hanson Attached for your review and comment are three versions of a Customer Service Survey which staff is proposing to implement. The first version is a general "Customer Service Survey" which can be used for all City Departments. This form would be distributed as follows: - Available at the front counter at City Hall; - Mailed to Customers as a result of telephone contact; - Given out at the time Certificates of Occupancy for building permits are issued, as well as various permits such as plumbing, mechanical, grading and excavation, fill, etc.; - Mailed to land use applicants with notification of Council action on their particular proposal; - Mailed to businesses along with annual licenses (liquor, gambling, etc.); - Public Works to carry in their trucks to distribute to the public they come in contact with; - Message on Cable TV and the City's Info Line that Customers can call to have a Survey Form mailed. The second version is a "Parks Maintenance Survey ". This form would be given to customers at the time a park usage permit is issued. It will also be mailed to a representative sampling of ball teams, coaches, etc. This form will be available at the front counter of City Hall for anyone who would like to pick one up. The third version is a "Parks & Recreation Program Evaluation ". This form will be given to coaches to distribute to parents some time toward the end of the activity. Staff is recommending that "Business Reply Mail" be used so that these forms can be folded, stapled, and returned by mail with no Page 95
