Hanson <br />Attached for your review and comment are three versions of a <br />Customer Service Survey which staff is proposing to implement. <br />The first version is a general "Customer Service Survey" which <br />can be used for all City Departments. This form would be <br />distributed as follows: <br />- Available at the front counter at City Hall; <br />- Mailed to Customers as a result of telephone <br />contact; <br />- Given out at the time Certificates of Occupancy <br />for building permits are issued, as well as <br />various permits such as plumbing, mechanical, <br />grading and excavation, fill, etc.; <br />- Mailed to land use applicants with notification of <br />Council action on their particular proposal; <br />- Mailed to businesses along with annual licenses <br />(liquor, gambling, etc.); <br />- Public Works to carry in their trucks to distribute <br />to the public they come in contact with; <br />- Message on Cable TV and the City's Info Line that <br />Customers can call to have a Survey Form mailed. <br />The second version is a "Parks Maintenance Survey ". This form <br />would be given to customers at the time a park usage permit is <br />issued. It will also be mailed to a representative sampling of <br />ball teams, coaches, etc. This form will be available at the <br />front counter of City Hall for anyone who would like to pick one <br />up. <br />The third version is a "Parks & Recreation Program Evaluation ". <br />This form will be given to coaches to distribute to parents some <br />time toward the end of the activity. <br />Staff is recommending that "Business Reply Mail" be used so that <br />these forms can be folded, stapled, and returned by mail with no <br />Page 95 <br />
