minnesota department of health <br />717 s.e. delaware st. minneapolis 55440 <br />To Whom It May Concern: <br />PLEASE FORWARD THIS LETTER TOGETHER WITH THE ENCLOSED INFORMATION <br />TO YOUR COMMUNITY WATER SUPERINTENDENT. <br />We have enclosed for your water superintendent a three month supply <br />of bacteriological report forms. A report form must be completed and <br />returned with every monthly water sample submitted to the Minnesota <br />Department of Health laboratory. All four copies of this form <br />should be returned with the sample. Directions for completing the <br />form and for collecting the water sample are located on the back of <br />the last (blue) copy. PLEASE READ THE DIRECTIONS CAREFULLY BEFORE <br />PROCEEDING TO COMPLETE THE FORM. <br />For those community water suppliers who are submitting monthly water <br />samples to laboratories other than the State laboratory, please <br />remember that you have a legal responsibility to have your samples <br />analyzed by an approved laboratory. Minnesota Rule MHD 147H states <br />that only samples which have been analyzed by an approved laboratory <br />will be acceptable for purposes of complying with the Minnesota <br />Safe Drinking Water Act. You should check monthly with the laboratory <br />to which you send your water samples to be sure that they are still <br />on the approved list. <br />Now that the Safe Drinking Water Act standards have gone into effect <br />you might want to inform your customers and the local media of the <br />possible impact the new rules could have. For your information, we <br />have enclosed a sample letter to the media entitled "Establishing <br />Media Contacts" and two sample fact sheets for possible enclosure <br />with water bills.
