REGIONAL OFFICE Dear • • In accordance with information submitted to this approval is hereby given for disposal of demolition waste on property described as office on cubic yards of during the period from to to the following conditions: 1. Disposal of garbage, dead animals, containers, leaves, yard trimmings, ashes, tires sludges, industrial wastes, vehicles, machinery, ous wastes is strictly prohibited. 2. Open burning shall be prohibited. 3. Demolition wastes may not be deposited in surface or ground waters. 4. Dumping of the demolition wastes shall be confined to as small an area as practicable and with appropriate facilities to confine possible wind- blown material within the area. At the conclusion of each day of operation, all wind -blown material resulting from the operation shall be collected and returned to the area by the owner or operator. 5. The demolition wastes shall be compacted as densely as practicable and covered with a minimum of six inches of compacted, suitable cover material on at least a weekly basis, or as specified by the Director. All previously filled areas shall be maintained with at least six inches of suitable cover material. 6. There shall be an available supply of suitable cover material which, if necessary, shall be stockpiled and protected for winter operation. 7. The demolition landfill shall be constructed and cover material graded so as to promote surface water runoff without excessive erosion. 8. Surface water drainage shall be diverted around and away from the landfill operating area. 9.
