10. Effective means shall be taken, if necessary, to control flies, rodents <br />and other insects or vermin. <br />11. Adequate dust control on the site shall be provided. <br />12. Equipment shall be available for adequate operation of the site. The <br />equipment shall be provided with adequate safety devices and adequate . <br />noise control devices. <br />13. Equipment shall be provided and <br />operation to control accidental <br />local fire protection agency to <br />needed. <br />kept at the site during the hours of <br />fires, and arrangements made with the <br />immediately acquire their services when <br />14. Adequate communication facilities shall be provided for emergency purposes. <br />15. The site shall be adequately screened by existing or provided means. <br />16. An attendant shall be on duty when the site is open. <br />17. Access to the site shall be controlled. A gate shall be at the entrance <br />to the site and kept locked when an attendant is not on duty. <br />18. The property owner(s) shall properly complete the Agency operational <br />report form and submit it monthly to the Regional Director. <br />19. Within one month after final termination of a site, or a major part thereof, <br />the area shall be covered with at least two feet of compacted earth material, <br />graded to a minimum 2% slope to promote surface water runoff without exces- <br />sive erosion. <br />20. The finished surface of the filled area shall be covered and maintained <br />with adequate topsoil and seeded to provide suitable vegetation immediately <br />. upon completion, oh immediately in the spring on areas terminated during <br />winter conditions. If necessary, seeded slopes shall be'covered with <br />straw or similar material to prevent erosion. <br />21. Prior to completion of a demolition landfill site, the Agency shall be <br />notified in order that a site investigation may be conducted by the Agency <br />staff before earthmoving equipment is removed from the property. <br />22. After completion of a demolition landfill site, a detailed description, <br />including a plat, shall be recorded with the county register of deeds. <br />The description shall include general types and location of wastes, depth <br />of fill and other information of interest to future landowners. <br />23. If the completed site is to be cultivated, the integrity of the finished <br />surface shall not be disturbed by agricultural cultivation activities. <br />If cultivated, a sufficient depth of cover material to allow cultivation <br />and to support vegetation shall be maintained. <br />050 <br />
