To implement this amended section, we propose the enclosed form be <br />attached to the regular license application as soon as possible, and the <br />language used on the form be incorporated in the license application by June <br />30, 1988. <br />Please file the workers' compensation insurance information form with the <br />license application. The Special Compensation Fund will request a copy of <br />the form if it is needed for a compliance investigation or uninsured <br />workers' compensation claim. If your agency's normal procedures would not <br />facilitate locating and providing a copy of the form upon request, please <br />contact the Special Compensation Fund so an alternate procedure can be <br />arranged. <br />If you have questions or suggestions on ways to improve this procedure, <br />please contact Leo M.
