Discharge report sheets included with this approval shall be completed <br />thoroughly and submitted to the MWCC quarterly. Laboratory reports for <br />required sampling and analysis conducted on the discharge during the <br />reporting period shall be submitted with the quarterly report. Each <br />report shall be postmarked no later than the 30th day following the end <br />of the quarter. Quarters run Jan. - March, April - June, July - Sept., <br />and Oct. - Dec. The first report shall cover the time period from <br />January 1 - March 31, 1989. Any additional sampling of this discharge <br />conducted beyond the requirements of this approval shall also be included <br />along with the report. <br />8. The MWCC shall receive notice: <br />- when changes are made to the proposed discharge plan <br />- within 48 hours after the discharge has temporarily or permanently <br />been discontinued <br />9. This discharge is subject to all applicable provisions of the MWCC Waste <br />Discharge Rules (5900.1600- 5900.7500) and the MWCC Strength Charge <br />System. <br />10. Discharges that exceed three years in duration will be subject to Service <br />Availability Charges (SAC). <br />11. This discharge approval is not exclusive. The approval does not release <br />you from conditions of the Minnesota Pollution Control Agency, Minnesota <br />Department of Health, Minnesota Department of Natural Resources, and the <br />City of Little Canada. <br />The MWCC reserves the right to inspect the pumpout /disposal system at any time. <br />The MWCC also reserves the right to revoke the approval of this discharge at any <br />time. Reasons for revocation include, but are not limited to: <br />- Noncompliance with approval conditions <br />- Adverse effects of the discharge on the MOS or local sewers <br />- Non - submittal or late submittal of quarterly reports <br />- Failure to make the required notifications (Item #8) <br />If the discharge is to extend beyond the expiration date, a letter requesting an <br />extension shall be submitted 30 days prior to the expiration date. The next <br />request for extension shall include data demonstrating the feasibility of alter- <br />native methods of disposal, (via an NPDES permit). <br />Page 5 <br />
