Northwest Suburban Youth Services <br />1910 West County Road B • Roseville, MN 55113 • General Information 636-5/48/ <br />PI' <br />,y t. <br />�:. <br />RECOGNITION <br />FOR SERVICE TO YOUTH <br />Northwest Suburban Youth Service Bureau invites you to nominate individuals <br />for our Outstanding Service To Youth Award. <br />This annual award honors individuals who provide outstanding service or <br />activities for the benefit of area youth (see communities below). Nominees <br />must recently or currently provide a volunteer service or activity to youth <br />which is above and beyond the individual's employment responsibilities or <br />provide exceptional service to youth within the scope of their job duties. <br />The award categories from which recipients may be chosen are: <br />LAW ENFORCEMENT: Individuals working within or retired from the field of <br />law enforcement, corrections and probation. <br />EDUCATOR: Individuals working within or retired from the educational <br />system. <br />COMiMUNITY <br />VOLUNTEER: <br />Individuals from within a group, civil /service clubs or <br />family. <br />YOUTH: Area youth between the ages of 12 and 18. <br />Award recipients will be honored at a ceremony to be held during the first week <br />in November. Recipients must be available to attend the ceremony. <br />Individuals and organizations may nominate candidates for these awards.
