Exhibit A <br />PROPOSAL CONTENT <br />As a minimum, your proposal shall include: <br />1. Firm name including business address, contact person, and <br />telephone number. <br />2. Year established. <br />3. Number on staff at this time. <br />4. Names of principals and key personnel with experience of each, <br />years of membership in firm, and type of specialization. <br />5. The name, experience, and qualifications of the person who will <br />be responsible for the management and administration of a <br />contract with the City. <br />6. The qualifications, resumes, and a listing of recent assignments <br />of the professional staff that will be assigned on an ongoing <br />basis to City projects. (If more than one person will be <br />assigned, please designate the primary ongoing contact.) <br />7. The availability of the aforementioned staff, including all <br />present ongoing assignments. <br />8. A listing of previous municipal clients including projects <br />worked on, specific activities performed, and the name of a <br />person who may be contacted at the municipality. <br />9. A list of current clients, projects being worked on, and the <br />name and phone number of a contact person. (We would prefer an <br />emphasis on municipal clients.) <br />10. A current rate schedule including <br />billable services. This schedule <br />each billable engineering service <br />applied if any.
