Confirmation that no officer or member of the <br />governing body of the organization has been <br />convicted of a felony or a gross misdemeanor <br />within five (5) years immediately prior to the <br />issuance of the license or has ever been <br />convicted of a crime involving gambling or has <br />had a license issued by the Board or Director <br />revoked for a violation of law or Board Rule <br />regulating lawful gambling. <br />7. Confirmation that no director, employee or <br />consultant of the organization has been convicted <br />of a felony or a gross misdemeanor involving <br />theft, dishonesty or false statement, or <br />involving the sale or possession of a controlled <br />substance, within five (5) years immediately <br />prior to the issuance of the license or has ever <br />been convicted of a crime involving gambling or <br />has had a license issued by the Board or Director <br />revoked for a violation or law or Board Rule <br />regulating lawful gambling. <br />8. A copy of an audit by an independent certified <br />public accountant for the twelve month period <br />immediately preceding the application. <br />9. A list of the names and addresses of all <br />financial institutions used by the licensee for <br />the deposit or withdrawal of charitable gambling <br />proceeds along with the name and address of the <br />account holder, account number, and type of <br />account held at the financial institution. <br />(B) Upon receipt of the materials required by Paragraph <br />(A), and not later than sixty (60) days from receipt <br />of notice from the Gambling Control Board, City staff <br />shall investigate the applicant, and based upon said <br />investigation, the City Council shall act on the <br />application. <br />(C) The Gambling Control Board shall be notified of the <br />approval of a premises permit or bingo hall license by <br />copy of the City Council Resolution approving the <br />premises permit or bingo hall license. Failure to <br />receive a majority affirmative vote of the entire City <br />Council shall constitute a denial of the application. <br />Page 11 <br />
