who has been asked to provide such a statement shall be <br />allowed to return to work until they comply with this <br />provision. <br />2. To receive credit for sick leave, an employee must <br />communicate with his immediate supervisor not later <br />than starting time on the day of illness, or as <br />provided by departmental rules. Employees are also <br />responsible for keeping their supervisors advised of <br />their illness status to remain eligible for sick leave, <br />unless they are hospitalized. <br />3. After all accrued sick leave is used, vacation leave <br />may be used, and payment made thereof, to the extent <br />the employee is entitled to such leave. <br />4. Sick leave will be allowed in case of personal illness <br />or physical incapacity of an employee or with the <br />demonstrated need of the employee's spouse or children. <br />5. Employees may use two (2) days of their annual sick <br />leave in hourly increments for doctor or dentist <br />appointments for their own medical and dental health <br />care as well as for their children. Documentation of <br />time used shall be furnished to the employee's <br />supervisor on the approved form. <br />6. Accumulated sick leave cannot be transferred from one <br />employee to another without approval of the City <br />Council. <br />7. An employee who makes a false claim of sick leave will <br />be subject to disciplinary measures, up to and <br />including discharge. <br />19 <br />Page 51 <br />
