.afa -RING I O(I -11T-C <br />November 8, 2013 <br />File No, 120164E -0071 <br />Honorable Mayor and City Council <br />City of Little Canada <br />515 Little Canada Road E. <br />Little Canada, MN 55117 -1633 <br />rant NOM <br />RE: 2013 STREET & UTILITY IMPROVEMENTS PROJECT <br />PARTIAL PAY ESTIMATE #5 & CHANGE ORDER #1 <br />Dear Council Members: <br />Please find attached 3 copies of Partial Pay Estimate No. 5 and Change Order #1 for the <br />above - referenced project. As you are aware, the Contractor for the project is Frattalone <br />Companies. The total amount earned by the Contractor on this partial pay estimate is <br />$938,820.96. Subtracting 5% retainage of $46,941.05 and previous payments of <br />$866,803.41, the amount due the Contractor is $25,076.50. <br />The Change Order addressed changed conditions on both Spruce Street and Pinetree <br />Ponds. On Spruce Street more poor material was removed than anticipated and a <br />sanitary and water service required relocation that was not included in the plan. On <br />Pinetree Pond there was a large cottonwood tree that hung over an adjacent property <br />owner's garage that required a crane and special equipment for removal to ensure that it <br />did not fall on the structure in the future. There was also construction debris buried on <br />the site that was not anticipated in the original scope. The remainder of the payment is <br />for pond work and additional erosion control, We have inspected the work performed by <br />Frattalone Companies, Inc. and find it to be in compliance with the plans, specifications, <br />and contract documents.
