No. SA No. 2 <br />Project Location: Little Canada Road, City of Little Canada, Ramsey County <br />Local Project No.: 120156E <br />Contract No.: <br />Local Agency: Little Canada <br />Contractor: FPI Paving Contractors, Inc. <br />Address/City/State/Zip: 3230 Rice Street, St. Paul, MN 55126 <br />Total Supplemental Agreement Amount $ 31,556.30 <br />This contract is between the City of Little Canada and the Contractor as follows: <br />WHEREAS: This current project work includes a mill and overlay of Little Canada Road <br />through the Interstate ramp area up to the bridge deck; and <br />WHEREAS: The County would like to modify the signal system and striping in the area <br />to accommodate the addition of designated left turn lanes on the Little <br />Canada Road bridge; and <br />WHEREAS: A portion of the signal and striping work will have impacts within the <br />existing project limits; and <br />WHEREAS: The City Engineer and County Engineer determined that the signal and <br />striping work to accommodate the left turn lanes on the bridge should be <br />completed with this project to avoid future impacts to the improvements <br />already being completed. <br />NOW, THEREFORE, IT IS HEREBY MUTUALLY AGREED AND UNDERSTOOD <br />THAT: <br />1. The Contractor agrees to complete the above described changes and additions <br />to the project. <br />2. This Agreement does not change any of the contract provisions unless otherwise <br />identified in this Agreement. <br />3. By signing this Agreement, the Contractor acknowledges receipt of the new plan <br />sheets for the signal and striping work, which are incorporated into this <br />Agreement by reference. <br />4. Payment for the work was reviewed and approved by the Engineer. <br />5. The Contractor will not make claim of any kind or character for any other costs or <br />expenses that he may have incurred or that he may hereafter incur in performing <br />the work or furnishing the materials of this agreement. <br />6.
