c. What are your other sources of funding? We will be requesting CDBG funding from other cities in Anoka County as well as from area service organizations. d. What other Anoka County cities will you be requesting funding from and amounts requested? We will be requesting funding from Blaine ($7,500), Ham Lake ($5,000), Ramsey ($2,000), and Andover ($3,500), 5. a. What are the goals of your agency? Rise goals are to plan, develop, and operate cost-effective and comprehensive vocational rehabilitation programs for people with mental, physical, developmental, emotional, and learning disabilities. b. What will the funds requested be used for? Will it be used for a new program, expansion of existing services, or to maintain service for Lino Lakes residents? Funds requested will be used to match a grant Rise received from the Minnesota Department of Transportation (MN DOT) for two mini -vans. The State will provide 80% of the funding and Rise is required to pay the remaining 20%. Funds received will be used in Rise's transportation program to replace aging vehicles in its system. The MN DOT grant will purchase two mini -vans, one will accommodate 13 passengers and one wheel chair and the other will accommodate 17 passengers and two wheel chairs. c. How many clients from the City of Lino Lakes are currently being served? Rise transports two Lino Lakes residents and five others from adjacent cities of Centerville, Circle Pines, Mounds View, and Shoreview. d. How many additional City of Lino Lakes clients will be served over the present level as a result of CDBG funding? All Lino Lakes residents who have requested transportation services from Rise are transported to their jobs each day. e. What benefits will be gained by Lino Lakes residents as a result of your program? Rise helps people living in Lino Lakes secure employment through job training and placement programs, affordable housing, and supported living services. Rise also provides transportation to program participants who are unable to access other generic forms of transportation. Rise picks people up at their homes, takes them to their jobs and returns them home at the end of the day.
