6. CLIENT POPULATION: <br />a. What portion of the client population is served by your program? <br />In 1993 Rise served over 1,300 individuals: Anoka County, 687 or 52%, <br />Hennepin County, 435 or 33%, Ramsey County, 143 or 11%, and Chisago <br />County, 15 or 1%. The number of people Rise served each day in 1994 <br />was 759. Rise transports 250 persons daily. <br />b. What is the income level of the clients to be served? <br />All Rise clients are low income. <br />c. What negative impact, if any, will occur on the target population if funding <br />is not granted by the City? <br />Rise will continue to serve all individuals needing transportation services <br />from Lino Lakes and other cities in Anoka County even if funding is not <br />approved by the City, however, Rise wants to provide transportation to its <br />program participants safely and efficiently at all times. If all the cities Rise <br />approaches for support, approve the requests at the estimated amounts, <br />Rise will be able to meet the match requirements and receive the vehicles. <br />In replacing and expanding the vehicles in its transportation program, Rise <br />is able serve a growing population that continues to need Rise program <br />services. <br />7. ACCESSIBILITY/AVAILABILITY: <br />a. How do you determine who is to be served? <br />All the people Rise serves are referred to the agency by Anoka, Hennepin, <br />and Ramsey Counties as well as the Department of Rehabilitation Services <br />(DRS). <br />b. What are your hours of operation? <br />8:00 a.nl. to 5:00 p.m., Monday thru Friday <br />c. What is your program capacity? Are you at capacity? <br />Rise doesn't have an actual program capacity. It responds to people in the <br />community with disabilities who need assistance in finding a job, housing, <br />and independent living services. <br />8. COORDINATION: <br />a. Is your agency providing a one -of -a -kind service, or are there similar <br />services being provided in the Anoka County area? <br />Rise is the only agency in this geographic area providing community-based <br />employment services. <br />
