5. a. What are the goals of your agency? <br />"The mission of ACCAP is to be a catalyst in creating tools & <br />opportunities for, and in empowering, lower income Anoka County <br />residents to achieve their aspirations & dreams. The Senior <br />Outreach Program achieves this goal by identifying and providing <br />referral, advocacy and case management services to vulnerable & <br />isolated seniors & disabled adults. The Outreach Worker assists <br />clients in accessing services to help their remain in their <br />homes in a safer and healthier manner through nutritional, <br />medical and social programs. <br />b. What will the funds requested be used for? Will it be used for <br />a new program, expansion of existing services, or to maintain <br />service to Lino Lakes residents? <br />The funds requested will be used to maintain Senior Outreach <br />efforts in the City of Lino Lakes. The Outreach Worker is <br />becoming much more a case manager, rather than just a referral <br />agency. Clients, due to multiple problems, are requiring <br />additional contacts throughout the year, and are facing <br />increased crisis situations. <br />c. How many clients from the City of Lino Lakes are currently being <br />served? <br />During the 1993-1994 program year, the Senior Outreach Worker <br />served approximately 50 clients, making an additional 21 follow <br />up visits. All seniors and disabled adults are eligible for <br />services. <br />d. What is the impact of your services? <br />With increased population, ACCAP anticipates additional clients <br />will be seen in the 1995-1996 program year. Many residents are <br />living much longer, but with reduced incomes and increased <br />needs. <br />e. What benefits will be gained by Lino Lakes residents as a <br />result of your program? <br />Because the Senior Outreach Program links clients to various <br />services and programs, seniors and disabled adults are able to <br />live longer and healthier lives in their homes. Referrals to <br />programs and services include food shelves, Medical Assistance, <br />home delivered meals, Lifeline, Public Health Nursing, Energy <br />Assistance Program (EAP), Telephone Assistance Plan (TAP), <br />Nutrition Assistance Program for Seniors (NAPS), among others. <br />When they receive these services, they benefit directly. <br />ANOKA COUNTY COMMUNITY ACTION PROGRAM, INC. <br />
