5. a. What are the goals of your agency?
"The mission of ACCAP is to be a catalyst in creating tools &
opportunities for, and in empowering, lower income Anoka County
residents to achieve their aspirations & dreams. The Senior
Outreach Program achieves this goal by identifying and providing
referral, advocacy and case management services to vulnerable &
isolated seniors & disabled adults. The Outreach Worker assists
clients in accessing services to help their remain in their
homes in a safer and healthier manner through nutritional,
medical and social programs.
b. What will the funds requested be used for? Will it be used for
a new program, expansion of existing services, or to maintain
service to Lino Lakes residents?
The funds requested will be used to maintain Senior Outreach
efforts in the City of Lino Lakes. The Outreach Worker is
becoming much more a case manager, rather than just a referral
agency. Clients, due to multiple problems, are requiring
additional contacts throughout the year, and are facing
increased crisis situations.
c. How many clients from the City of Lino Lakes are currently being
served?
During the 1993-1994 program year, the Senior Outreach Worker
served approximately 50 clients, making an additional 21 follow
up visits. All seniors and disabled adults are eligible for
services.
d. What is the impact of your services?
With increased population, ACCAP anticipates additional clients
will be seen in the 1995-1996 program year. Many residents are
living much longer, but with reduced incomes and increased
needs.
e. What benefits will be gained by Lino Lakes residents as a
result of your program?
Because the Senior Outreach Program links clients to various
services and programs, seniors and disabled adults are able to
live longer and healthier lives in their homes. Referrals to
programs and services include food shelves, Medical Assistance,
home delivered meals, Lifeline, Public Health Nursing, Energy
Assistance Program (EAP), Telephone Assistance Plan (TAP),
Nutrition Assistance Program for Seniors (NAPS), among others.
When they receive these services, they benefit directly.
ANOKA COUNTY COMMUNITY ACTION PROGRAM, INC.
