In addition, the cost of running the Senior Outreach Program is <br />quite low when compared to the high financial cost to taxpayers <br />and service providers when seniors and disabled adults are <br />placed unnecessarily or prematurely in a long term facility. <br />The financial burden of all community members is eased when <br />clients are able to remain in their homes and contribute <br />socially and ecomonically to the community. <br />6. CLIENT POPULATION: <br />a. What portion of the client population is served by your <br />program? <br />The Senior Outreach Program serves seniors aged 60 and over, <br />with particular focus placed on low/moderate income and/or very <br />elderly individuals. It also serves disabled adults of any age <br />who need assistance in obtaining services. - <br />b. What is the income level of the clients to be served? <br />All seniors and disabled adults are eligible to be served. Most <br />clients in the City of Lino Lakes (75% last year) had incomes <br />which were less than the 135% Poverty Guidelines. <br />c. What negative impact, if any, will occur on the target <br />population, if funding is not granted by the City? <br />The Senior Outreach Program cannot be implemented without <br />funding from the City of Lino Lakes. Necessary programs such as <br />energy assistance, weatherization, food programs and others will <br />be difficult for the elderly and disabled to access without <br />assistance or advocacy. This assistance in many cases provides <br />emergency intervention; in other cases it provides help before <br />the situation becomes so serious that illness or injury can <br />occur. <br />7. ACCESSIBILITY/AVAILABILITY <br />a. How do you determine who is to be served? <br />Any senior or disabled adult is eligible, however the Outreach <br />Worker seeks out low-income, frail, or isolated persons. ACCAP <br />receives referrals from hospitals, social workers, Anoka County. <br />churches, Public Health Nursing, community members and leaders. <br />neighbors, family members, and other agencies, as well as the <br />seniors themselves.
