What are the hours of operation? <br />ACCAP administration is open 8:00 a.m. - 4:30 p.m. Monday <br />through Friday, however the Outreach Worker works varying hours <br />depending upon the client. <br />c. What is program capacity? Are you are capacity? <br />The Outreach Worker currently works 20 hours per week in the <br />Quad Cities, which is at capacity. <br />8. COORDINATION: <br />a. Is your agency a one -of -a -kind service, or are similar services <br />being provided in the Anoka County area? <br />To our knowledge, there are no similar services in Anoka County <br />or in the City of Lino Lakes which provide outreach involving <br />case management, referral services and advocacy to vulnerable <br />seniors and disabled adults. <br />b. If other area agencies provide similar services, how do you <br />screen for duplication or overlap of services with other <br />agencies? <br />N/A. <br />9. STAFFING: <br />a. What is the total number of staff for the contracted program? <br />One Senior Outreach Worker for the City of Lino Lakes, working <br />8.5 hours per week (20 hours total in the Quad Cities). <br />b. What are the credentials of staff providing services under the <br />contract? <br />The present Senior Outreach Worker is in her 3rd year of <br />service. She is knowledgeable about the community and its <br />resources. She is a member of the Senior Issues Network <br />and meets every two weeks with 5 other Outreach Workers and the <br />Coordinator of Senior Programs and Volunteer Services at ACCAP <br />for training. An annual allowance is provided for the <br />professional training and education regarding issues involving <br />seniors, the disabled and the services provided to them. Daily <br />supervision and guidance is provided by the Coordinator of <br />Senior Programs. Professional development is encouraged at all <br />levels. <br />c. What is your client/staff ratio? <br />Every household is seen one on one by the Outreach Worker. It is <br />estimated that the Outreach Worker will serve 50 clients this <br />program year, making the ratio 50 clients per .2 FTE. <br />ANOKA COUNTY COMMUNITY ACTION PROGRAM, INC. <br />
