AGENDA ITEM #6A <br />STAPP ORIGINATOR Mary Kay Wyland <br />OATS July 18, 1996 <br />TOPIC <br />96-25, N edCenters Managed Care Facility, <br />Apollo Drive, Conditional Use Permit, Site <br />Plan Review and Preliminary Plat. <br />MedCenters Managed Care (HealthPartners) would like to construct a <br />9,200 square foot clinic on Apollo Drive, just south of Lilac. The <br />property is Zoned General Business and a medical clinic requires a <br />conditional use permit in the GB Zoning District. <br />SITE PLAN REVIEW: <br />SETBACKS: The proposed structure will be located toward the rear <br />of the lot well within the setback requirements. visitor parking <br />will be off Apollo Drive in front of the building withjemployee <br />parking behind the building on the 35W side of the site. <br />BUXWING EXTERIOR The building exterior <br />decorative brick with brick accent.. <br />BUILDING HEIGHT: The <br />height, 45' is the <br />Di <br />will be a <br />ht allowed <br />rdinance requires a minimum <br />OFF-STREET PARKING: A total Of 50 patient/visitor parking spaces <br />and 16 employee spaces will be provided for a total of 66 spaces. <br />Handicapped spaces shall comply with ADA Rules and Regulations <br />CE'RCULATION AND ACCESS: Access to the site will be provided by a <br />*fid driveway off Apollo Drive. A park dedication fee will be required as a part of <br />this development as subdivision of the land is involved. <br />LIGHTING: The City's Community Service Officer has reviewed the <br />lighting plan and recommended approval with some minor changes. <br />LOADING AND TRASH HANDLING: The trash handling area is indicated <br />on the site plan and shown as a screened enclosure. <br />SIGNS: A].I signage must conform to the City's Sign Ordixance and <br />proper permits obtained prior to any installation. <br />UTILITIES,, <br />the site of <br />E AND GRADING: Utilities are availabieEto serve <br />Drive.
