State of Minnesota -U.S. Bank Commercial Card Solutions -Participating Addendum <br />Contract# 75413 Purchasing Card Contract# 75427 Fleet Card <br />Appendix A, Document 4 -U.S. Bank Product Descriptions <br />U.S. BANK PRODUCT DESCRIPTIONS <br />1. U.S. Bank Purchase Card. The U.S. Bank Purchase Card is a charge card designed for use by <br />Cardholders to charge goods and services related to the business activities of Customer. U.S. <br />Bank also provides central purchase accounts, which can be used for the same purpose, but <br />without the issuance of a physical card. <br />2. U.S. Bank One Card. The U.S. Bank One Card is a charge card designed for use by Cardholders <br />that combines Corporate and Purchase Card capabilities on a single Account. The One Card can <br />be utilized for travel and entertainment related expenses as well as to charge goods and services <br />related to the business activities of Customer. <br />3. U.S. Bank Fleet Card. The U.S. Bank Fleet Card on the Voyager Network is a Corporate Liability <br />charge card designed for purchases of motor fuels and other products and services by fleet vehicle <br />operations using a transaction processing, reporting and payment system established by U.S. Bank. <br />4. U.S. Bank Managed Spend Card. The U.S. Managed Spend Card is a specialized corporate <br />liability card with declining balance functionality designed for use by Customers or Cardholders to <br />charge business related goods and services. Customer may also elect to have Cards issued to <br />family members relocating employees. Unless requested for a different duration, Managed Spend <br />has a term of twelve (12) to thirty-six (36) months. U.S. Bank also provides Managed Spend <br />Central Billing Accounts, which can also be used for business or relocation expenses. Based on the <br />credit worthiness of Customer and/or its Cardholder, U.S. Bank, at its sole discretion, shall establish <br />a credit limit of no less than five hundred U.S. Dollars ($500.00). <br />5. U.S. Bank Emergency Response Card. The U.S. Bank Emergency Response Card is a corporate <br />liability charge card designed for use by Cardholders under unusual or special circumstances such <br />as disaster relief/recovery efforts. Customer provides the Emergency Response Card to <br />Cardholders to charge goods and services against a Corporate billed account. <br />6. U.S. Bank Corporate Card. The U.S. Bank Corporate Card is a charge card designed for use by <br />Cardholders to charge travel, entertainment and other goods and services that are related to the <br />business activities of Customer. U.S. Bank also provides central travel accounts, which can be <br />used for the same purpose, but without the issuance of a physical card. <br />Page 26 of 37
