• Tax Parcel Number: 34-31-22-34-0001, 34-31-22-43-0006, 34-31-22-43-0007, 34-31-22-43-0008, 34- <br />31-22-43-0010, 34-31-22-44-0001, 34-31-22-44-0006, 34-31-22-44-0008, 35-31-22-33-0002. <br />At a minimum attach each of the following to the EAW: <br />• County map showing the general location of the project; (See Appendix A — Figure 1) <br />• U.S. Geological Survey 7.5 minute, 1:24,000 scale map indicating project boundaries (photocopy <br />acceptable); (See Appendix A — Figure 2) <br />• Site plans showing all significant project and natural features. Pre -construction site plan and <br />post -construction site plan. (See Exhibit A) <br />• List of data sources, models, and other resources (from the Item -by -Item Guidance: Climate <br />Adaptation and Resilience or other) used for information about current Minnesota climate trends <br />and how climate change is anticipated to affect the general location of the project during the life <br />of the project (as detailed below in item 7. Climate Adaptation and Resilience). <br />6. Project Description: <br />a. Provide the brief project summary to be published in the EQB Monitor, (approximately 50 <br />words). <br />The North Oaks Company LLC is proposing construction of a mixed -use development <br />located in the City of Lino Lakes, Anoka County, Minnesota. The project will include the <br />construction of senior housing, market -rate apartments, affordable housing, commercial <br />space, and single-family lots. <br />b. Give a complete description of the proposed project and related new construction, including <br />infrastructure needs. If the project is an expansion include a description of the existing facility. <br />Emphasize: 1) construction, operation methods and features that will cause physical <br />manipulation of the environment or will produce wastes, 2) modifications to existing equipment <br />or industrial processes, 3) significant demolition, removal or remodeling of existing structures, <br />and 4) timing and duration of construction activities <br />The Project will consist of the construction of a new mixed -use development. This will consist of a <br />combination of single family residential homes, townhomes, apartment complexes, senior living, and <br />retail space. <br />1) Construction activities that will manipulate the environment will include tree removals, <br />site preparation and earthwork, utility installation, and the construction of new <br />structures including housing units and retail stores as well as the construction of roads, <br />walks/trails, and stormwater ponds. See Appendix B for the current site plan of the <br />project. Construction wastes will be generated during the project construction. <br />2) N/A, there are no existing equipment or industrial processes located within the project <br />area. <br />3) N/A, there are no existing structures located within the project area. <br />4) Site preparation work is anticipated to begin as soon as spring 2025. Site construction <br />is anticipated to take place over the course of multiple phases which will occur over the <br />course of several years dependent upon market conditions. <br />Page 2 <br />
