LAKE MANAGEMENT, INC. <br />Minnesota Operations <br />10400 1851'' Street North <br />Marine on St. Croix, MN 55047 <br />(651) 433-3283 Phone Fax # (651) 433-5316 <br />Email: infoWakemanagementinc.com <br />PROPOSAL FOR AQUATIC NUISANCE MANAGEMENT PROGRAM 2025 <br />Submitted to: <br />Date: September 11, 2024 <br />Mary Sell <br />564 Lonesome Pine Trail Phone: (612) 802-0877 <br />Lino Lakes, MN 55014 Email: mkaysell@msn.com <br />Lake Management, Inc. proposes to furnish at its expense, all of the necessary materials, subject to state and <br />federal regulations, equipment and labor as well as to direct and administer treatment as needed for the control <br />of the excessive growth of submerged and floating aquatic weeds and algae in the water area described as follows: <br />2025 Aquatic Management Program — <br />Pond (2.5a) located at 564 Lonesome Pine Trail, Lino Lakes, MN <br />Seasonal Maintenance Program......................................................................$2,038.00 <br />Two -week inspection/application intervals, mid -May through mid -September. <br />Includes phosphorus binding applications for nutrient reduction. <br />Bio-augmentation applications included for sediment reduction and water clarity. <br />Aquatic colorants/dyes included and applied upon request. <br />Lake Management uses only EPA approved, Minnesota DNR allowed products. <br />In consideration of the goods and materials furnished, use of equipment and services rendered, payment is to be: <br />Two Thousand Thirty -Eight and 00/100 Dollars ($2,038.00) <br />Terms: Net April 1, 2025 <br />Note: Our seasonal contract begins approximately May 15`h and ends September 15'. If applications are required <br />outside of seasonal contract dates or an additional treatment is requested, a minimum $249.00 charge may apply. <br />Questions about our program or special requests should be directed to: Mike O'Connell at (651) 433-3283. <br />In rare cases, large amounts of decomposing algae could cause a reduction in dissolved oxygen, placing fish <br />at risk. While loss of fish is extremely rare, Lake Management Inc. cannot be responsible for such an <br />occurrence. Please notify Lake Management if fish are present. <br />Lake Management, Inc. carries insurance for both liability and property damage and workers' compensation <br />and will, upon request, confirm that coverage by certificates issued to the customer. <br />This proposal may be withdrawn by Lake Management, Inc. if not accepted within 30 days. <br />ACCEPTANCE OF PROPOSAL <br />The above payment and conditions are satisfactory and are accepted.
