Page 20 <br /> <br />the employee or beneficiary pays the full cost of coverage at the City’s group rates plus any <br />additional administrative fee, if applicable. <br />The Human Resources Department will provide each eligible employee with a written notice <br />describing rights granted under COBRA when the employee becomes eligible for COBRA <br />coverage plan. The notice contains important information about the employee’s rights and <br />obligations. <br />5.4 Deferred Compensation <br />The City will provide regular full-time employees the opportunity to participate in a pre-tax <br />retirement savings program through payroll deductions. This voluntary retirement saving plan is <br />designed to allow tax savings for employees. <br />Full-time employees will be permitted to receive a cash payment for 50 percent of their unused <br />annual Sick Leave/Earned Sick and Safe Time accrual exceeding 720 hours at the conclusion of <br />each calendar year. The balance of the unused Sick Leave/Earned Sick and Safe Time will be <br />added to the employee’s Sick Leave/Earned Sick and Safe Time bank up to the maximum <br />accrual of 960 hours. The cash payment will be paid at the conclusion of each calendar year into <br />the employee's designated deferred compensation plan as authorized by the City. <br />5.5 Long-Term Disability <br />Long-term disability insurance is provided to regular full-time employees Long-term disability <br />benefits are available based upon the terms and conditions contained in the contract with the <br />long-term disability insurer. Terms and benefits under the program are subject to change from <br />time to time. <br />5.6 Safety Committee <br />The City of Lino Lakes recognizes its responsibility to provide the safest conditions for its <br />employees by ensuring all reasonable safeguards are established in the workplace. Employees <br />are responsible for observing all safety rules and for using all available safety equipment. <br />Employees are also responsible for reporting any unsafe condition to their supervisor. The <br />efficiency of any municipal operation can be measured directly by its ability to control <br />unnecessary loss. To that end, the Lino Lakes Safety Committee was established. The Safety <br />Committee is made up of employees who meet and examine the City’s policies and procedures <br />and review accident and injury reports. The Safety Committee also reviews and updates the <br />City of Lino Lakes Safety Policies and Rules. <br />5.7 Employee Wellness <br />Employees may convert accrued Sick Leave/ESST or PTO/ESST leave to pay for their monthly <br />health club membership dues or online subscriptions up to a maximum of $60 per month, <br />provided that the employee maintains a Sick Leave/ESST balance or PTO/ESST balance of 80 <br />hours. <br />Page 81 of 166
