P49 <br />The Finance Department will provide each eligible employee with a written notice describing <br />rights granted under COBRA when the employee becomes eligible for COBRA coverage plan. <br />The notice contains important information about the employee's rights and obligations. <br />5.4 Deferred Compensation <br />The City will provide regular full -time and part-time employees the opportunity to participate in <br />pre -tax retirement savings program through payroll deductions. This voluntary retirement saving <br />plan is designed to allow tax savings for employees. <br />Full -time employees will be permitted to receive a cash payment for 50 percent of their unused <br />annual sick leave accrual exceeding 720 hours at the conclusion of each calendar year. The <br />balance of the unused sick leave will be added to the employee's sick leave bank up to the <br />maximum accrual of 960 hours. The cash payment will be paid at the conclusion of each <br />calendar year into the employee's designated deferred compensation plan as authorized by the <br />City. <br />5.5 Long -Term Disability <br />Long -term disability insurance is provided to regular full -time and part-time employees. Long- <br />term disability benefits are available based upon the terms and conditions contained in the <br />contract with the long -term disability insurer. Terms and benefits under the program are subject <br />to change from time to time. <br />5.6 Safety Committee <br />The City of Lino Lakes recognizes its responsibility to provide the safest conditions for its <br />employees by ensuring all reasonable safeguards are established in the workplace. Employees <br />are responsible for observing all safety rules and for using all available safety equipment. <br />Employees are also responsible for reporting any unsafe condition to their supervisor. The <br />efficiency of any municipal operation can be measured directly by its ability to control <br />unnecessary loss. To that end, the Lino Lakes Safety Committee was established. The Safety <br />Committee is made up of employees who meet and examine the City's policies and procedures <br />and review accident and injury reports. The Safety Committee also reviews and updates the City <br />of Lino Lakes Safety Policies and Rules. <br />5.7 Employee Wellness <br />Employees may convert accrued sick leave or PTO leave to pay for their monthly health club <br />membership dues up to a maximum of $60 per month, provided that the employee maintains a <br />sick leave balance or PTO balance of 80 hours. <br />13 <br />
